Liability Release Forms

On all trips/courses with Deep Sea Divers Den, each person will be asked to fill out and sign certain release forms.

Below you will find links to copies of these forms you will be asked to fill out (all links open in a new window).

Credit Card Authority form - please use this form if you are not the credit card holder.

PADI Courses

If you are taking any of the following courses, you will be asked to fill out and sign the forms below:

• PADI Open Water Course (learn to dive)*
• PADI Advanced Open Water Course
• PADI Adventure Diver Course
• PADI Rescue Diver Course
• Medical Statement (276 KB)
• Safe Diving Practices (20 KB)
• Liability and Release (19 KB)
  

Please note, if you answer YES to ANY of the questions on the Medical Statement, you may be required to have a diving medical to Australian Standard AS4005.1. This must be prearranged if you are taking the Advanced Course, Adventure Diver or Rescue Courses, at least one day before your course is due to start.

*Open Water Courses must have an Australian Dive Medical to AS4005.1 (can be done on the first day of your course at the dive centre) see below.

If you are doing an Open Water Course (Learn to dive):

A dive medical to Australian Standard AS4005.1 will be required. We have a Doctor on site who will organise your medical on the first day of the course.

All dive medicals from overseas will NOT be accepted unless they state they are to Australian Standard AS4005.1

Generally most students pass the Medical Examination without any difficulty. There are however, some CONTRA -INDICATIONS for scuba diving. If you have ever suffered from any of the following conditions, you are unlikely to pass the medical examination here:

• PNEUMOTHORAX
• EPILEPSY
• HEART SURGERY
• DIABETES REQUIRING INSULIN
• ASTHMA
• Or if you are currently PREGNANT

Certified Divers

If you are a certified diver on OceanQuest or ReefQuest, you will be asked to fill out and sign the following form:

Snorkellers

If you are a snorkeller on OceanQuest or ReefQuest, you will be asked to fill out and sign the following form:

Introductory Diving

If you are doing an introductory dive on OceanQuest or ReefQuest, you will be asked to fill out and sign the following form:

Introductory Diver Form (275KB)
If you answer YES to any questions further medical clearance may be required. This must be arranged prior to day of departure.

Spirit of Freedom

If you are travelling on Spirit of Freedom you will be asked to fill out and sign the following forms. These forms should be filled out, signed and faxed to DSDD prior to departure.
